  • Rating of hair loss (severe, moderate, mild, or none)
  • Gender (M/F)
    • If female: pregnant or trying to conceive (y/n)
  • Heart condition (Free text or N)
  • Scalp condition (Free text or N)
  • Blood pressure condition (Free text or N)
  • Allergy to minoxidil (y/n)
  • Allergy to finasteride (y/n)

At the time of purchase (checkout), the following PI must be collected to fulfill your order:

  • First and Last Name
  • Shipping address
  • Company (optional)
  • Phone Number
  • Credit card information

Interacting with Our Third-Party Processors

Please note that we do not collect or process your credit card or cardholder data. We use Shopify as our third-party vendor, which is PCI-DSS compliant. Shopify collects and administers cardholder processing, including payment at checkout. We also integrate with [PayPal and Stripe].
